Supplements for Healthy Aging: What Works and What Does Not

Introduction

Supplements for healthy aging occupy an enormous market, and most of that market outruns its evidence. I have spent my career reading these trials, and the honest summary is uncomfortable for my own industry. A few interventions hold up. Many do not. Some carry real caution attached to them. In this article I walk through what the largest randomized trials actually found, in whom, and over how long. I cover the null results, the positive ones, and the safety signals. I also explain what I require before any compound earns a place in a formula I sell.

What Healthy Aging Means, and What It Does Not

Healthy aging describes maintaining function, independence, and quality of life as the years accumulate. The phrase does not mean stopping or reversing aging, and no supplement has been shown to do either. Researchers usually measure healthy aging through function, e.g., cognition, muscle strength, mobility, and freedom from disability. Those outcomes matter more than any laboratory marker because they determine what your daily life actually looks like. I want that framing settled before discussing supplements for healthy aging because it sets the standard for evidence. A compound earns attention when it improves function in people, not when it changes a number in a dish.

Supplements for Healthy Aging: What the Largest Trial Found

The largest test of the most popular supplements for healthy aging produced a result the industry rarely quotes.

Manson and colleagues randomized 25,871 US adults, men aged 50 and older and women aged 55 and older (Manson et al., 2019). Every participant was free of cancer and cardiovascular disease at enrollment. Vitamin D at 2,000 IU daily produced no reduction in cancer or cardiovascular events in those healthy adults. The median follow-up in that trial ran 5.3 years, so the null result is not a matter of insufficient time. The same trial tested marine omega-3 at one gram daily in those adults, and found no reduction either. I state that plainly because vitamin D and fish oil are among the most purchased supplements for older adults. Correcting a documented deficiency is a separate question entirely, and your clinician can test you for one. Women in that age range have a further question to weigh, which I address in brain health for women over 50.

Where the Evidence Turns Positive: Multivitamins and Memory

Not every finding in this literature runs negative, and one recent trial deserves your attention. Yeung and colleagues randomized 3,562 older adults to a daily multivitamin or placebo for three years (Yeung et al., 2023). The multivitamin group showed improved episodic memory at one year compared with placebo in those older adults. The authors estimated that improvement as equivalent to 3.1 years of age-related memory change. That is a modest effect measured on one memory test, and I will not oversell it to you. It is also a properly randomized trial in exactly the right population, which is more than most products can claim.

Anti-Aging Supplements and the Antioxidant Problem

Products sold as anti-aging supplements lean heavily on antioxidants, and the trial record in this area is concerning. Bjelakovic and colleagues pooled 78 randomized trials covering 296,707 participants, both healthy adults and patients with chronic diseases (Bjelakovic et al., 2012). Among the trials at low risk of bias, beta-carotene and vitamin E appeared to increase mortality. Vitamin C and selenium showed no significant effect on mortality in that pooled population. The reviewers found no evidence supporting antioxidant supplements for primary or secondary prevention.

More antioxidant is not better, and the form tested matters. The ATBC trial used synthetic all-racemic alpha-tocopherol rather than the natural RRR form, and natural alpha-tocopherol is more bioavailable in humans (Ranard & Erdman, 2018). Ranard and Erdman also concluded that few studies have compared the two sources directly, so no firm conclusion about their relative health effects is available yet. My own view is that the synthetic form is a real part of the explanation. Isolated high-dose antioxidants also behave differently from antioxidants delivered in whole food.

Antioxidant defense and inflammatory signaling are difficult to separate, which I examined in my article on inflammation and brain health. I raise this knowing it complicates the marketing of much of my own industry, including compounds people expect me to praise.

Why Protein and Exercise Outrank Any Vitamins for Healthy Aging

Ranking these interventions honestly puts food and movement above any pill, including the ones I formulate.

Campbell and colleagues reviewed protein requirements in older adults and supported intakes of 1.0 to 1.6 grams per kilogram daily (Campbell et al., 2023). Protein intake below the recommended allowance worsens age-related loss of muscle size, quality, and function in older adults. Muscle loss with age is one of the clearest threats to independence, and protein is the lever that moves it.

Exercise carries the strongest evidence of any behavior for maintaining cognition as you age. Northey and colleagues pooled 36 randomized trials in community-dwelling adults older than 50 (Northey et al., 2018). Aerobic exercise, resistance training, multicomponent training, and tai chi all improved cognitive function in those adults older than 50. I apply that same exercise evidence to one audience in brain health for men over 50.

No supplement substitutes for adequate protein and consistent movement, and anyone selling you otherwise is not being straight with you.

How I Choose Supplements for Healthy Aging

People ask me to name the best supplements for healthy aging, and my answer usually disappoints them. After all of that, a fair question follows. Why sell supplements at all? My answer is narrow. I am interested in compounds that the modern diet almost never supplies.

Polysaccharides (i.e., long chains of sugar molecules that act as signaling compounds rather than fuel) fall into that category. Do you know anyone who eats aloe vera? I do not! Supplements for healthy aging deserve far more skepticism than the market gives them. Vitamin D and omega-3 produced no reduction in cancer or cardiovascular events in healthy older adults. Antioxidant supplements carry a mortality signal worth respecting. A daily multivitamin improved memory modestly in older adults, and protein and exercise outrank every pill on the shelf.
